PARENTS this is an OPT OUT waiver – You only need to sign this waiver if you do not want your student to appear on camera as part of class-wide Capitol Classroom Zoom meetings.

This form is for participation in Capitol Classroom, CONNECTS or any Teach With TVW recorded event during the current school year. We appreciate your participation!
